I have a Pacchioni V-system double in B flat (bought second hand). It has two tuning holes on the higher chamber: one is a tiny one in the side, the other is larger and on the tip of the right end. The point appears to have been ground off leaving a small concavity with the hole in its centre.

It plays in tune, and the surface at that point is smooth and shellacked like the rest of the ocarina, or I'd think it had just been chipped off.

Does Pacchioni normally do that?

Yes, it is normal on multi-chambered ocarinas by Pacchioni. The quadruple I have has three such holes on the tip. It's as good a place as any for a tuning hole.
